Is there a written test you have to take for the Commercial Multi-Rating?
Or is it just the checkride and oral test?

No written for the multi engine ratings. Whether you have a private or commercial. There isn’t even a written for the MEI.

Well, yes, you’d have to take the Commercial written and pass the commercial practical exam, but nothing multi-engine specific, as I think was the intent of the OP’s question.
