In-Flight Medical Kit

As an anesthesiologist who has responded to some interesting in-flight medical issues on commercial airlines, who determines what is in the medical kit? The airline or the FAA?

I have sometimes felt like I was being handed a Playschool Medical kit and the stocked drugs are OK but could be improved in type or amount.

The FAA has the basic list, airlines may add to it as they see fit.
