On a scheduled departure time, say 12:30pm, does that mean push from gate time or off runway time? What do pilots consider it?

If you’re talking about FlightAware, it means wheels off time (see Questions/Answers on top right of all pages).

If you talking about what’s shown in an airline’s timetable then you are talking about push back from the gate.
