I forget if its the head honcho or his son’s plane, in this case it may be both. Anyways they are there for believe it or not one of the most prestigious medical facilities in the world the Mayo Clinic. Talk about a hell of a drive to the doctor. 
The Royal Flight does not have 777’s, it would be “chartered” to the Government from the Government owned airline. The Gulfstream’s would also be chartered from Saudia’s Special Flight Department.

The Mayo Clinic is in Minnesota. But they travel there ALL the time and stay for months. Rumor has it they leave store shelves at Mall of America empty

I actually think they meant to say that they fly into CLE for the Cleveland Clinic - one of the top heart hospitals in the world. There’s several other threads on here regarding this.
