Include airlines that existed the day you traveled. For instance if you have been on Air Cal before the AA buyout and American more recently go ahead and include both. I realize PSA and PSA are two different airlines but include both if you have had that privilege. If you know the airline flying as one of the express carriers use that name, otherwise use what is written on the side of the fuselage.
I'll start, in no particular order.

Golden State
Hughes Air West
Allegheny
Swift Aire
North Central
Republic
National
Delta
TWA
Western
Pan Am
PSA (original)
American
Sky West
Alaska
Wings West
Horizon
Mesa
Air Canada
Eastern
British Airways
Lufthansa
Air France
Turkish
Saudi Arabian
United
Continental
Imperial
KLM
Swissair
Cascade
Piedmont
Southwest
Crossair

wow, more than I thought off the top of my head.

How'd you get on that World Airways flight? I thought that was a charter company.

Short history of World:
- started out as a charter airline
- operated a few schedule routes late 70's and early 80's
- became all charter again

For a timetable of World from June 1980, go to http://damiross.net/airlines.html and scroll down until you see "World Airways" on the left.

A more detailed history can be found at Wikipedia: http://en.wikipedia.org/wiki/World_Airways

I flew Icelandair YYZ -> KEF -> ARN -> KEF -> YYZ. I didn't have anything to complain about except for the fact that the 757 seating got a little cramped on the trip westbound and for Denver it can only be farther. The IFE was pretty good, everyone has seat back TVs. Iceland is a beautiful country, I'm very glad I went after my conference in Stockholm.

Yes, the 737-200 was unforgettable - I was only 9 but I'll never forget my first plane ride, it was from Toronto to St. Petersburg Florida for a March break vacation.

Some flights/airlines are long lost to memory. For instance, my first flight ever as a young lad was a DC-3 that my Dad and I flew from Columbus (OH) to Chicago. I clearly remember the flight but the airline? Not a chance!
I've also never had, or taken rather, the opportunity to fly across the pond. I'd love to do that sometime. And Iceland is high on my wish list. However, since you asked for lists, not reminiscences, here is my list......

Aero Virgin Islands
Air BC
Air Canada
Air Wisconsin
Alaska
Allegheny
America West
American
Braniff
Comair
Continental
Delta
Eastern
Frontier
Horizon
Mesa
Mesaba
Midway
Nordair
North Central
Northwest
Ozark
Piedmont
Pinnacle
Republic
Sky West
Southwest
Sunbird
TWA
United
US Air
Western Pacific

Haven't heard of either of these in a long time.

For those who don't know, Apache was a commuter airline in Arizona. The CJ-600 was a turboprop conversion of the De Havilland DH-104 Dove.

The crash of a CJ-600 in 1971 was the demise of Apache. The FAA had grounded the aircraft. This was the only aircraft type operated by Apache and they were unable to afford new aircraft.

These are the Airlines and Aircraft that I recall (to the best of my memory):

Air Canada - 707
Air Jamaica - DC9
Air New England (prob. Twin Otter)
Air Wisconsin- Shorts “box”
American - DC10/Convair 990
Allegheny-BAC 1-11
American Eagle CRJ (I think)
Braniff - 727
Capitol - Vickers Viscount
CNAC - C46
Continental - DC7
Delta - L1011/Convair 880
Eastern - DC8
Frontier - 737
Hawaiian - Convair 340
Hub Airlines (Twin engine prop- into IND)
MATS - C54/C97/ C121
National - 727
North Central - DC9
NWA - Martin 202/404/Electra II/B720/B727
Ong - Cessna 420 (Meigs Field to MKE)
Ozark (original) - F27
Pan Am - Stratocruiser & 747
Piedmont - “Yakasuki 11"
PSA (original) DC9
Republic - Convair 340/540/DC9
Southern - 727
Southwest - 737s
TWA - Connies, DC9, 707s
United - Caravelle - DC6B - 727
Unknown: (Cedar Rapids to ORD) - DC3
Unknown (Pittsburgh to Ottowa, ON)
Western - Electra II

In all my airline travels between 1948 and today, I've had:
(1) Only Ozark ("Krazo" spelled backwards) completely lost my luggage.
(2) Three engine failures, two on TWA aircraft and one on NWA
(3) One aborted take off: Eastern Air Lines at IND
(4) One aborted landing: Delta going into BOS
(5) One scary as #@** landing at Portland, ME
(6) One "turn-around" because of a sick passenger (PHL to DEN)

The quitest aircraft (in my opinion): Convair 880 & 990
I can honestly say that I've never had a rotten flight, although some flights were less pleasant than others.
I have met and spent time with some extremely interesting people (e.g., the Surgeon General of the Army).
To the best of my recollection, the FAs on my non-military flights of course, have all been polite.

Finally, in general, my zillion miles on on airplanes have never been boring!

Interesting list. Just one possible correction: Air Canada didn't fly 707's but DC-8's. If this was Canadian Pacific, it could have been a 707 because they flew those for a short time.

I didn't put it down on my original list but I have also flown on a Convair 880 of Delta. This was before I started keeping track of my flights. Can't really tell if it was quiet or not inside. My dad had been rotated back from Germany. All I remember of the flight is that it was late at night and it had been a long day. We had departed Rein Main (spelling) that morning on a Trans Caribbean DC-8 (also not on my listed), stopped in Bangor, Me, and continued on to CHS AFB. The it was a flight to ATL followed by another one to MSY (this was the 880), and finally another Delta flight into IAH.

RW:

It certainly could have been a DC-8. What made the trip from YYC to LAX memorable:

1) After a day of meetings I hurried to the airport and got absolutely the last seat (middle one, of course) on the plane.

2) The person sitting by the window had a small laptop computer. It was the first time I'd ever seen one.

3) It was an international flight and the adult beverage(s) were free. That, by itself, made the middle seat much more "comfortable."

Thanks for the correction, I appreciate it.
